Instructions

Please record the behaviour that your son or daughter shows at the moment (over the last three months). Please describe and rate the most usual way he/she displays this behaviour.

Each question is followed by a list of alternatives. Please select the option that best describes the behaviour shown by your son or daughter.

For those items that ask about frequency, please rate how frequently your son or daughter might display the behaviour over the course of the day if you were watching them all day.

Please try to complete each question as accurately as you can and try not to leave any question unanswered.
